Fill out the google form below to request a progress report, completion, or termination report. All requests will be completed in 5 -7 business days free of charge. All expedited requests for reports under 48hours cost $5 for processing fees. Payment is due prior to the receipt for expedited reports.

Court Group Intake Request

This form is for anyone who needs to take a court-mandated class. Fill it in and we will schedule you. How to prepare for an intake: 1. Bring ID (driver's licence). 2. Zoom/phone intakes: WiFi and an email account you know the password to. A laptop is preferred; an iPhone may work, Android does not. In-person intakes: an email account with its password. 3. Payment: $100 per intake or $100 re-enrolment — be prepared for your card to be charged. Then $55 per week (DV Level 1, Elder Abuse, CAT, Anger Management) or $80 per week (DV Level 2). 4. Time: intakes take about 1–1.5 hours.
